Does HMB do anything for natural lifters?
If you're already lifting and eating enough protein, HMB has essentially nothing to add on top of creatine and protein. It does a real, modest job protecting muscle that's shrinking during bed rest, illness, or older age, just not muscle that's already growing.
HMB (beta-hydroxy-beta-methylbutyrate) is a metabolite your body already makes in small amounts when it breaks down the amino acid leucine. Take it as a supplement and you get far more than digestion alone provides, and the pitch has stayed consistent since the 1990s: HMB slows the machinery that breaks down muscle protein, mainly the ubiquitin-proteasome pathway, so more of what you build sticks around. That mechanism is real. The question is whether it matters once you’re already lifting.
It matters more, or less, depending on how much muscle breakdown is happening in the first place. A beginner’s muscle, a bedridden older adult, or someone recovering from illness or surgery is losing protein faster than a well-trained lifter whose synthesis and breakdown are already running close to balanced. HMB has more to intercept in the first group than the second.
Where it earns its keep
The clean wins show up outside the weight room. A meta-analysis splitting trained from untrained lifters found a real, if modest, gain in leg strength in men who were new to training. A 2025 meta-analysis in adults over 50 found small but statistically significant increases in lean mass and handgrip strength. A separate 2025 umbrella review of meta-analyses found HMB’s benefits concentrate in older adults and people losing muscle to illness or disuse, not healthy young trained people.
Where it stalls out
Pull trained subjects out of those same datasets and the numbers go flat. That same trained-vs-untrained meta-analysis called the effect on trained lifters trivial. A 2020 meta-analysis of young adults following resistance-training programs found no significant benefit for fat-free mass or one-rep-max strength over placebo. Even the International Society of Sports Nutrition’s own 2024 position stand, generally sympathetic to the supplement, admits trained-athlete data are mixed and that combining HMB with creatine is additive at best, not synergistic.
If you’re already training hard and getting enough protein, HMB isn’t the missing piece. Put the money toward creatine instead. It has earned its keep in trained lifters in a way HMB hasn’t.
