Do exogenous ketone esters actually improve performance?

The short answer

Mostly not. One famous lab study kicked off the whole ketone-ester industry, but most independent controlled trials since have found no performance benefit, and a few show athletes going slower and feeling worse.

The pitch behind ketone esters is genuinely elegant. Ketone bodies like beta-hydroxybutyrate are a fuel your liver already makes during fasting or a strict low-carb diet, and the theory holds that they’re a slightly more oxygen-efficient fuel than carbohydrate. Drink a concentrated dose before or during a race, the logic goes, and you hand an endurance athlete a clean extra fuel tank that spares muscle glycogen for the finish. It’s a tidy story, and it built a real industry around a drink most athletes describe as tasting like jet fuel.

The theory got its momentum from a single 2016 Cell Metabolism trial that reported a roughly 2% time-trial improvement in cyclists who drank a ketone ester with carbohydrate. Worth knowing: the senior scientist on that paper held a financial stake in the company built to commercialize the same ester, which is exactly why independent replication mattered so much. Replication has mostly gone the other way. A 2022 meta-analysis pooling eight randomized trials found no consistent endurance benefit from ketone esters or precursors, and the field’s most complete review concluded that most performance and recovery studies show trivial or no effect. In the most direct real-world test, professional cyclists given a ketone diester before a time trial rode slower than on placebo, not faster.

The gut problem

Ketone esters hit the stomach hard. In that same professional-cyclist trial, every rider reported gastrointestinal discomfort — nausea, reflux, dry retching — and one rider couldn’t complete the time trial at all because of vomiting and dizziness. That’s a brutal tradeoff in a sport where the whole skill is not feeling sick while working hard for hours, and symptoms get worse at the higher doses needed to meaningfully raise blood ketones.

Recovery and focus claims are earlier-stage

The performance case is close to settled. Recovery and cognitive claims are younger and genuinely unresolved: reviewers describe the post-exercise recovery signal as preliminary at best, and cognitive-performance data as scarce and mixed. Worth watching, not worth planning a race around.

Skip it unless a coach has a specific, tested reason to try it — and test it in training long before you’d ever consider it on race day.
